How It Works

You’ll spend time with Dr. Zablozki-Amir to review your medical history and health goals. You’ll receive a holistic and practical perspective, with guidance on your next steps in your health action plan.

You can use the information you’ve learned to incorporate on your own or with your health team, or continue to work with the doctor in various ways to create and implement a proactive plan plan and support your goals. The doctor can help you decide, based on your consultation time together which would be best for you.

Functional Medicine Roadmap

A Strategic Plan to Optimize Health

A comprehensive strategy with advanced testing to identify concerns and optimize your health. Work with your doctor on a personal, actionable plan.

Cost

$1,500 plus testing and lab fees.

How It Works

You’ll continue to work with Dr. Zablozki-Amir over the next 4 visits, during a 3-4 month time period, to create a comprehensive action plan unique to you, with a holistic, preventive, root cause perspective, to understand and improve your health.

*Pricing includes visits with the doctor as well as the work she does related to your care before, after and between visits such as supplement prescriptions, lab analysis, record review, reports, collaboration with other practitioners, checks ins, action support, etc.

How It Works

You’ll partner with Dr. Zablozki-Amir for ongoing support and guidance. From targeted concerns to tracking your health, or collaborating with other practitioners, you’ll have support.

For those who want to address a specific targeted concern that would only require limited lab testing or periodic oversight right from the start …and… for those who’d like to continue beyond the initial roadmap program.

*Hourly rates take into account the appointment time as well as the work done by the doctor on your behalf, before and/or after the visit, to help optimize your care.
